The NHS does not fund the services listed below. Fees are set to reflect the time, expertise, and administrative work involved. Payment may be required in advance.

Medical Reports

• Detailed medical report: from £60
• Insurance or employer medical report: from £80
• Cancellation of holidays form: £35
• Armed Forces medical questionnaire: £65

Forms & Certificates

• HGV/Taxi medical (excluding ECG/bloods): £120
• Adoption/Fostering medical (adult): £120

Other Services

• Subject access request (above standard NHS provision): £30
