Who do I see?
Requesting an Appointment
We are a total triage practice, which means all appointment requests are managed through our online health form. This allows us to understand your concerns fully and make sure you receive the right care as quickly as possible. Every form is carefully reviewed by a GP, and you will always receive a response with clear next steps. We offer appointments as urgent on the day, clinically important but not urgent and routine. Appointments can be face to face, phone call or where necessary video consultations or home visits.
If you need help completing the form, our team is here to support you. You can call us on 0151 260 9119, or you are welcome to come into the surgery where a member of staff will assist you in filling it out if you are unable to wait on the phone.
Please note online health forms are only available Monday – Friday 7:30am – 6:30pm. Outside of these hours you will not be able to submit an online health form. If you need help outside of these online hours please contact 111.
From 1st April 2026 the majority of our communication must be via email as per Cheshire and Merseyside ICB requirements. Please ensure you check your emails for any response you may be given. Please ensure our email communication is not diverting to your junk or spam folders.

Urgent Appointments
If you feel your problem is urgent, you should still contact us using our online health form. If you present to the surgery as a walk-in we will still require a health form to be submitted for triage. We can support you to fill this form in for you at reception. All requests are reviewed promptly by our clinical team to ensure you receive the most appropriate care as quickly as possible. We offer appointments as urgent on the day, clinically important but not urgent and routine. Appointments can be face to face, phone call or where necessary video consultations or home visits.
As part of our total triage system, every request is assessed individually. However, it is important to understand that GP practices are not an emergency service and are not equipped to manage life-threatening conditions.
A medical emergency is a situation where immediate care is needed to prevent serious harm or death. These situations are not suitable for GP consultations or online health forms.
Call 999 or attend A&E immediately if you experience:
- sudden chest pain with collapse or severe breathlessness
- signs of stroke (facial droop, arm weakness, speech difficulty)
- loss of consciousness or seizures
- severe allergic reactions with breathing difficulty or swelling
- uncontrolled bleeding
- severe head injury
- acute confusion or sudden behavioural change
- suspected sepsis
- severe trauma or accidents
If your condition is urgent but not life-threatening, you can also contact NHS 111 for advice and guidance on the most appropriate care.
If you are unsure, it is always safer to seek emergency care.
For all other urgent medical concerns, please complete our online health form and we will assess your request and respond with the most appropriate next steps.
Cancelling your appointment
We understand that things come up, but unattended appointments prevent others from being seen and place extra pressure on our overstretched NHS. Each missed GP slot costs around £30 on average—across England more than 15 million GP appointments are wasted per year, costing the NHS well over £400 million and equating to more than 1.2 million GP hours lost annually—reducing access for patients who need our care.
Meanwhile, GP services are under immense strain: since 2015, over 1,400 GP practices have closed in England, leaving fewer doctors to serve more patients adding to waiting times and travel burdens.
If you are unable to attend a future appointment, please let us know as soon as possible so we can offer the slot to another patient in need.

Home Visits
Home visits are available for patients who are genuinely unable to attend the surgery due to illness or medical need. The decision to provide a home visit is made by a clinician and is based on what is safest and most appropriate for your care.
If you feel you may need a home visit, please contact the practice as early as possible, ideally before 10:00am.
When we can offer a home visit
- If you are truly housebound due to illness, frailty, or disability
- If your condition makes it unsafe or impossible for you to come to the practice
- In some cases, if you are temporarily unwell following surgery or illness
When a home visit may not be appropriate
- If you are able to leave your home for other activities (e.g. shopping, hairdressers, holidays or hospital appointments)
- If transport is the main issue – we encourage use of family, friends, or community transport
- For children, who are usually best assessed at the surgery or in hospital if very unwell
Important information
- Home visits are not a right and cannot be requested on demand
- The clinician will decide the most appropriate type of appointment (phone, video, surgery, or home visit)
- Visits will be allocated to the most appropriate clinician, which may include a paramedic, occupational therapist, nurse, or GP
- In emergencies (e.g. chest pain, severe breathlessness, collapse), please call 999 immediately
Keeping our staff safe
To allow us to provide care safely, we ask that you:
- Keep pets in another room
- Do not smoke during the visit
- Let us know about any access or safety concerns in advance
We are committed to providing safe, fair, and high-quality care to all patients. Home visits are reserved for those who need them most, ensuring we can care for everyone effectively.
