Sick (fit) notes
A doctor’s medical certificate is not required for the first six days of illness. If you are off work for less than six days you should fill in a self-certification form available from the DSS.
If you would like to request a fit note, please complete a request via our Paco DFD below, and select “Admin Requests” -> “I need a sick (FIT) note”.
If you feel you must have a doctor’s note for less than six days a private note can be provided, but please note that you will be charged a fee.
