💡 What is QOF?
QOF (Quality and Outcomes Framework) is a national NHS programme that guides GP practices on how to provide the best care.
It is developed using evidence from medical experts and national health organisations, and sets out the key areas we should focus on to improve patient health each year.
👉 It helps make sure all patients across the country receive consistent, high-quality care based on best practice.
QOF aims to have a unified approach across the country in tackling:

Over the next 12 months, we’re working to improve care in these key areas:

Making sure treatments are safe, effective, and reviewed regularly

Supporting people with heart failure and related conditions

- Stop smoking support
- Weight management
- Vaccinations
- Screening (like cervical screening)
🧠 Mental Health & Dementia
Regular reviews to support both mental and physical wellbeing
💬 Why might we contact you when you feel well?
You might hear from us for a review, test or check-up — even if you feel completely fine.
That’s because many conditions (like high blood pressure or diabetes) don’t always cause symptoms early on.
👉 Seeing you early helps us:
- Spot problems before they become serious

- Support you to stay healthy for longer
These appointments are just as important as coming in when you’re unwell.
